Cardizem and Cardizem LA (an extended release, once-a-day tablet form of diltiazem) are used to treat angina caused by exertion. Cardizem CD and Cardizem LA are also used to treat high blood pressure. Cardizem SR is used only for the treatment of high blood pressure.

Adults: The average daily dose is 180 milligrams (mg) to 360 mg, divided into 3 or 4 smaller doses.
Cardizem CD
Adults: This is a once-a-day form of Cardizem. For high blood pressure, starting doses range from 180-240 mg. For chest pain, doses range from 120-180 mg.
Cardizem LA
Adults: This is a once-a-day form of Cardizem. For high blood pressure, when used alone, doses start at 180-240 mg and may be increased to as much as 540 mg once daily. For chest pain, the starting dose is 180 mg once daily with increases every 7-14 days if needed.
Cardizem SR
Adults: The initial dosage is 60-120 mg taken 2 times a day. The usual dose is 240-360 mg a day.
